An affidavit ( A-fə-DAY-vət) is a written sworn statement of fact voluntarily made by an affiant or deponent under affirmation (an oath or) administered by a person authorized to do so by law. This is extremely helpful when patients have extensive complaints and you are trying to establish the veracity of symptoms. The physician must investigate and determine what limitations the patient self imposed upon themselves. The physician should also make a written statement of medically prescribed restrictions as applicable.
